Leave of Absence

How to Apply

To apply for a Leave of Absence, please fill out the "Request for Leave of Absence" Form in Frontline Central. You may access this form by signing in to your Frontline Central account below, clicking on "My Forms" and then "Forms I Can Start." Upon receiving an email from Frontline Central that your form has been reviewed you may then contact Human Resources to schedule a meeting; not all circumstances will require a meeting.
